CarePartners will hold a free and informative presentation about the essential aspects of Hospice and Advanced Care Planning. Advance Care Planning involves sometimes hard to navigate legal documents which allow you to give directions for your future medical care. This session is designed to help you understand these important documents so that your medical wishes are known in writing. It is important to have these directives at any age.
